SCAN grant provides more healthcare for seniors

Keeping seniors active, engaged, independent across SoCal

Jamboree receives SCAN grant for senior health
Senior residents at 61 Jamboree communities – including 13 exclusively senior communities - across Southern California will now have access to professional medical care from a mental health nurse, thanks to a grant funded by SCAN Health Plan, one of the nation’s largest not-for-profit Medicare Advantage plans. SCAN brings together experts in medicine, gerontology, psychology, and social services to fulfill their mission: keeping seniors healthy and independent. The $15,000 grant will provide professional medical oversight to ensure that Jamboree seniors are active and thriving.

The part-time mental health nurse will confirm that Jamboree seniors are accessing all of their benefits and receiving all available care and services needed to support them as they live independently and age in place. Additionally, the nurse will build the capacity of onsite property management and resident services staff, providing quarterly trainings and advising on case management for seniors with higher needs. This has the added benefit of providing support for staff who can defer to an expert in any given resident situation. Together, the collaboration will make it possible to better identify any issues with medications, mental and physical health, and propose potential new interventions to ensure our seniors stay healthy and housed.

Keeping senior residents physically active and mentally engaged is part of REACH, an acrostic that outlines our unique approach to senior resident services using five key values for customized services that enrich the lives of residents. Jamboree’s REACH approach not only encourages the health of seniors at our properties, but it also ensures that they thrive in their daily lives, with social, learning, and physical activities.
